Our online appointment scheduling system is provided as a convenience. Scheduled appointments are subject to availability and may be confirmed, rescheduled, or canceled by our staff. We request at least 24 hours' notice for cancellations. Repeated no-shows may result in scheduling restrictions at the clinic's discretion.

Uses and Disclosures for Treatment, Payment, and Healthcare Operations. Breach Notification. In the event of a breach of your unsecured PHI, we will notify you and the U.S. Department of Health and Human Services as required by law. Notifications will be provided without unreasonable delay and no later than 60 calendar days after discovery of the breach.
